This agreement and form are strictly for official use by CapitalBox. All collected information will be used solely to deliver consultancy services as per the client’s request. It shall not be shared, sold, or misused for any purpose outside CapitalBox operations.

Please fill the CapitalBox Support Request Form completely and accurately. Ensure all sections — personal information, selected support issue(s), and description — are clearly filled. Tick all relevant options that reflect your concern. If your issue is not listed, choose “Other” and write it manually. To avoid delays, attach any required documents such as ID proof, bank slip, screenshot, or prior communication.

Once completed and signed, scan or take a clear photo of the form and email it to yasb@capitalbox.in with the subject line: Support Form – [Your Name]. Also ensure your contact number and preferred mode of communication are correct. Our backend team will respond within 24–48 business hours. Avoid duplicate submissions unless your issue remains unresolved.
